Russia has thousands of nukes. It will take 5 to wipe out the UK. Nukes have a relatively small "total kill" zone which is big enough to wipe out many cities. But Russia is not going to target cities. Instead it will target strategic military assets, reducing the West's ability to fire back. Most of those are well away from cities, so they are not in "total kill" zone. Fallout also affects a massive area, but it's a concentrated zone downwind of the blast (downwind at the height of the Mushroom cloud, not ground level). After about a month, very sporadic "help" might start arriving, but only a fraction of what is needed, because the rest of the world will have more than enough problems of its own. "Camps" will be set up, which the gullible people displaced from fallout zones will go to in the expectation that "someone else will do something", but realistically, there will be almost no help and they will be worse than the worst third world famine camps and quickly people will be locked in. However, in rural areas that are unaffected by fallout, things could start looking as "normal" as you get when they are cut off by snow. But, cities will be a disaster area. There will be almost no food getting in. As for areas with fallout ... no help at all. Anyone who stays in one is certain to die.
