I'm not sure if the format for this checklist will import without garbaging up, but this is a list we use for civil defense volunteers performing damage assessment after storms, which might make a good "thought starter" to revise for your location and environment:

Personal Equipment Checklist For a 12-Hour Operational Period In the Urban Environment

Carry In Coat / Safety Vest / Cargo Pockets:

 Medical ALERT Info and ICE Contact Card
 Drivers license, Served Agency ID and FCC radio operator license copy
 Name tag attached to outer garment
 $5 coins for phone calls, vending, etc.
 WATER – 2 pint bottles on person as minimum
 SNACKS – for 12 hours
 PERSONAL MEDICATIONS for 24 hours
 SANITATION / COMFORT ITEMS; sunscreen, insect repellent, hand sanitizer, baby wipes, Chapstick, etc.
 Scout knife or Multi-tool, Leatherman, etc.
 Backup AAA flashlight +extra batteries
 Whistle – Fox 40 or ACR (distress signal)
 Compass, orienteering type on neck cord
 Police / Fire Dept. street / utility map book
 24,000 scale topographic of assigned search sector
 State DOT road map
 Bandanna, bright ICS color per assignment, visual locator flag signal
 Extra reading glasses, if you use them

Personal Protective Equipment (in SAR ruck)
 Safety glasses, ANSI Z87.1-1989
 TWO, N95-rated disposable single-use masks
 Reflective vest
 Work gloves
 Personal FIRST AID KIT, includes 4 pair of medical exam gloves
 Primary flashlight, Petzl AAA headlamp, on helmet for searches
 Extra set of AAA batteries + bulb for search light
 Hardhat + CERT rescue equipment, – per assignment

Clothing – Worn
 Sunglasses – 97% UV protection
 Boots, ankle support, traction sole
 Hat (Sun / Rain protective)

Extra Clothing – Carried (in SAR ruck)
 Rain anorak or poncho
 Wool / fleece warming layer for expected weather
 Extra dry socks, in Ziploc bag so they will stay dry

Communications:
□ 3G or later generation cell phone capable of receiving SMS text with GPS location enabled
□ phone mobile charger (or extra charged battery pack)
□ Air Band, Marine, Public Safety or Ham 2 meter band portable, per assignment
□ Extra charged battery pack or AA battery case for portable transciver
□ CB, FRS or GMRS portable per assignment, citizen / neighborhood watch coordination
□ Quick reference guide for each transceiver carried
□ Headset or earphone for portable transceiver
□ Speaker-microphone for transceiver
□ External 12VDC power cord for portable transceiver

Forms pack:
□ Notebook, pencil, pen, felt tip or grease pencil, and:
□ ICS 309 Log Forms (3)
□ ICS 213 Message Forms (10)
□ ICS 201 Incident Brief form (1)
□ ACS-RACES Quick Reference (1)

Shift Extended = 24 hours - Additional Emergency Supplies – Store In vehicle:

 COMPLETE CHANGE of dry clothes: shirt, trousers, socks, shoes, and underwear
 Extra drinking water to total one gallon
 Extra food for three meals, balanced nutrition
 Personal meds, hygiene, sanitation supplies for 24 hours
 Two wool blankets (or sleeping bag with liner and bivvy)
 Duct tape – multiple uses, emergency repairs
 Tent or tarp for rain / sun / wind shelter
 Tow strap or chain with hooks
 Tools: standard and Phillips screwdrivers, adjustable wrench, side-cutting pliers, shovel, hand saw or axe.
 Three highway flares or reflectors
 Fire extinguisher - 10 pound ABC multi-rated dry chemical
