When we think of spearfishing we tend to think of a spear with a single head.
Our ancestors added multiple 'tines' splayed outwards around the central shft of their spears.
You aimed directly where you thought the fish was and chances were one of your 'tines' would correctly jab the fish at some point.

Simple and effective.

Done it, tried it, failed! It really isn't a simple system. The spikes would have to be really splayed to use in water over 1 foot deep.

Another trick is to dip the bottom of the spear into the water, then using that as the reference point with which to angle for the fish.
